Additional Information For Price List
We are a small, fee-for-service clinic, meaning that we collect payment at the time of service (cash, credit card, check, flexible savings accounts and health savings accounts). After each session we will email an itemized bill to our patients so those who are interested in reimbursement can independently submit it to their insurance companies.
Our One-time Fee Policy
At Pro-Active Physical Therapy, we always take patient care as our top priority. To continue doing this, we bring access to the Digital ‘Home Exercise Program’ for all our patients. There will be a one-time fee of $25/Calendar Year (From Jan to Dec of the year) which will be charged at the time of check-in.
